Job Details Job Location: Spicewood - Austin, TX Position Type: 40 HRS - 1 FTE Salary Range: Undisclosed Job Shift: M-F 8A-5P Description JOB TITLE: Certified Nursing Assistant (CNA) DEPARTMENT: Clinical Services REPORTS TO: CNA Manager SUPERVISES: None POSITION SUMMARY The Certified Nursing Assistant (CNA) provides compassionate and high-quality personal care and support to patients in home care settings and within assisted living facilities, personal care homes, skilled nursing facilities, and independent living facilities. The CNA works closely with the hospice team, facility staff, patients, and families to ensure comfort, dignity, and quality of life. They are responsible for developing, maintaining, and supporting strong working relationships with facility partners to ensure seamless coordination of care. DUTIES AND RESPONSIBILITIES Facility-Based Patient Care • Assist patients with daily living activities (bathing, dressing, grooming, toileting, and mobility assistance) in assisted living, personal care homes, skilled nursing, and independent living facilities. • Provide compassionate companionship and emotional support to patients. • Observe and report changes in patient condition, behavior, or needs to the supervising nurse or facility staff. • Assist with feeding and hydration, ensuring dietary requirements and preferences are met. • Support safe patient transfers and positioning, including the use of wheelchairs, walkers, and mechanical lifts as needed. Coordination & Communication with Facility Staff • Develop and maintain positive working relationships with facility administrators, nurses, and care staff. • Communicate effectively with hospice nurses, social workers, and facility teams regarding patient needs and updates. • Attend care team meetings at assigned facilities to discuss patient care and support plans. • Coordinate with facility housekeeping and dietary teams to maintain a clean and comfortable patient environment. Compliance & Documentation • Accurately document patient interactions, care provided, and any changes in condition according to hospice and facility policies. • Follow all state, federal, and facility regulations regarding infection control, patient rights, and safety protocols. • Adhere to HIPAA regulations to ensure the confidentiality of patient information. Supportive & End-of-Life Care • Provide comfort measures and emotional support to patients and families during end-of-life care. • Assist in managing symptoms through gentle repositioning, hydration, and non-medical interventions. • Support families by offering education, reassurance, and presence during the dying process. • Assist in post-mortem care in accordance with hospice and facility protocols. Qualifications MINIMUM REQUIREMENTS • Current Certified Nursing Assistant (CNA) license in the state of Texas. • Currently listed in good standing on the Texas nurse aide/misconduct registry, with no finding against the aide relating to client abuse or neglect or misappropriation of client property, as verified through the Texas Department of Human Services (DHS). • CPR certification (or ability to obtain within 30 days of hire). • Valid driver’s license, auto liability insurance, and a safe driving record (if travel is required). • Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR). PREFERRED QUALIFICATIONS • Prior experience in hospice, assisted living, skilled nursing, or home care preferred. • Knowledge of hospice federal and state regulations preferred. • Strong communication and teamwork skills to collaborate with hospice and facility staff. • Ability to prioritize tasks and provide compassionate, patient-centered care. • Commitment to dignity, respect, and comfort for end-of-life patients. • Adaptability to various care settings and facility policies.
