Facilitates the development and maintenance of data collection, leveraging existing data to identify gaps in care and recommends areas of focus for quality improvement activities. Accurately collects and records specific data elements from patient medical records based on established criteria and guidelines. Performs data abstraction of clinical findings from medical records and data entry in clinical data systems. • DEPARTMENT DESCRIPTION • The system Quality Outcomes and Reporting team provides analysis of quality data, handles quality reporting and oversees CMS quality incentive programs for the system.
